  Note: P.O. Boxes are not an accepted address. If your address is not accepted please Google your address and use any corrected address.

• Email Address and Phone number for the primary contact 

• Your Church’s website (a Facebook link is ok if you don’t have a website)

Official Organizational Information:

• The Church’s EIN (aka the Federal Tax Number)

• Bank account number and Routing Number (your account can only be linked to one)

Legal Representative Info:

• Name and date of birth
  Note: The date format should match that of your country (e.g., MM/DD/YYYY in the US).

• A photo of their photo ID (most commonly drivers license in PNG or JPEG format)
